Credit Check Sheet of references (may include banks you deal with, vendors you pay on a regular basis, local businesses you deal with, credit card accounts)  
Your Tax ID Number, resale ID Number or Vendor Number  
How long your company has been in business  
Main contact person name, address, and phone number.

We only open wholesale accounts with retailers who have a vendor resale or Tax ID Number. Businesses who make payment via credit card can send their order with application for quick processing and delivery.

General Information: All orders are shipped UPS directly from the manufacturer.  Minimum wholesale order is $100.
